Severe pain after stent removal: How often does it happen and can anything prevent it?

Many patients complain of some pain after having a ureteral stent is removed. This pain is usually mild and self-limited. However, the pain can sometimes be severe. Because patients may not have been warned about the possibility of significant pain beforehand, it can come as an unwelcome surprise, and patients may be left wondering whether something is wrong. Patients who are affected by this pain may describe it as being worse than their original stone pain. In some cases the pain can be bad enough to bring them back to the emergency room.

Up to now, there hasn’t been much research on this topic and it was not clear how often this phenomenon occurs or what might be done to prevent it. A recent publication in the journal BJU International now suggests that severe pain after stent removal may occur in as many as half (55%) of patients. In the study, performed by researchers at Oregon Health & Science University and the Portland Veterans Administration Medical Center, patients having their indwelling ureteral stents removed were either given a placebo pill or a single 50mg dose of the non-steroidal anti-inflammatory drug (NSAID) rofecoxib fifteen minutes before their stent removal procedure. Patients were asked to report their pain just before and 24 hours after their stent removal on a scale ranging from 1 to 10, with 7 or above being considered severe pain. Six of the eleven patients (55%) given placebo developed severe pain after their stent removal while none of the ten patients given rofecoxib developed severe pain.

The authors speculated that the mechanism by which the NSAID medicine may have provided such a dramatic reduction in severe pain occurrence might have been through reducing ureteral spasms or by decreasing pressure within the kidney. While the results of the study are very encouraging, further research will be needed to confirm the findings. For now though, the study is currently the only evidence we have on how often post stent removal severe pain occurs and what we might do to prevent it – take a single dose of a NSAID pain medication fifteen minutes before removing a stent.

Of note, the medication used in the study, rofecoxib (brand name Vioxx), was withdrawn from the US market in 2004 due to concerns about cardiovascular side effects and it is no longer available. The authors report in their paper that they now use a single 220mg dose of the NSAID naproxen with their patients. Naproxen is available over the counter (as a generic or under brand names such as Aleve) but you should check with your doctor about whether you should take it and make sure to read the warnings from the manufacturer as some patients with certain medical conditions shouldn’t take it.

While you might think that a single pill of naproxen can’t be very strong, other research actually supports its use for acute pain. In 15 randomized studies involving 1509 participants, naproxen was found to be effective for relieving moderate to severe pain in patients after surgery. Half of patients who were given a single dose of naproxen experienced at least half pain relief and the effects of the medicine lasted on average for up to nine hours. I had a stent put in about a month ago when I went in for laser lipotripsy surgery. My ureter started to tear, so the surgeon stopped the surgery and left the stent in to dilate the ureter. I went back in for a second surgery Thursday, Aug. 23, and this time it was successful. He broke up and removed my 14 mm stone and put in a new stent with a string attached. I was supposed to remove it myself after 2-3 days, but after reading some of these posts, I’ve decided to have it done by the urologist. Thanks for all the feedback, folks. I hope you’re all feeling better.

Stent removal update: Well, after reading some of these posts, I decided to have a medical professional remove my uretral stent, even though my doctor had left instructions for me to do it myself. After lots of calls to various hospitals and clinics — including my family doctor — I was able to get an appointment with a urology nurse to take care of the procedure six days after surgery. As instructed, I took a Tramadol for pain about half an hour before my appointment. The nurse was very gentle and understanding and advised me to sit-stand against the exam table as she pulled the stent out. It was a pretty uncomfortable sensation (kind of like the buzzing feeling your sinuses sometimes get after a big sneeze), and I’m glad I didn’t have to do that myself. Also, she inspected the stent for evidence of blood clots or other problems and proclaimed it to be in excellent shape. I wouldn’t have known to check for those factors. Anyway, I immediately felt better, though my groin area was sensitized for about an hour. Now it’s been more than 24 hours since removal, and I’ve had no blood in the urine. I’ve only been urinating every three hours (as opposed to once or twice an hour with the stent) and I’ve been basically pain-free. I guess I’m one of the lucky ones (not to mention very blessed). Hang in there, folks. I know this stuff is not fun. Trust your doctor and do what works best to help you feel better.

OK folks – Scary stories all around! I had a Laser Lithotripsy done last Monday. Doctor put in a stent, and I was asked to take it out 3 days later (Thursday). This was a stent with a string, and I was able to pull it with minor discomfort, and very very happy to have gotten it out! As many have said, I felt good for about 10 minutes, and then the pain hit and I thought that was the end of me! Reached for he Tylenol #3 (with Codeine) that I was prescribed, and that did nothing. Finally figured out that Ibuprofen 800mg and Aleve seemed to do the trick. What also is helping is occasional heat. I was also taking flomax, and since they took the whole stone out, wasnt’ sure that it made sense to continue taking Flomax? (But I am not sure on this one). It is now Saturday (2 full days post stent removal) and pain still persists. It seems as though it comes and goes. It’s not consistent. Depends I think on how much medicine is in me. It seems I have to take medicines every 4 to 5 hours just to ensure I don’t “relapse” into that real bad pain. Doctor said that if this does not get better by Monday morning (2 days away), he needs to see me. Could be a UTI, could be something else, may need stronger pain meds. Anyone deal with such a situation? In short, is this “normal”? What should do or not do in terms of medicines, diet, heat treatment etc? What’s the magic formula??

Ive had 2 stents 12 hours ago. They were placed in 7weeks ago due to a serere bladder laceration. This happened during the hysterectomy from hell. So last 7weeks (mostly hospitalized) has been a painful nightmare. I had such high hopes of pain leaving after stents out. But no. 🙁 im in agony. Ita so wrong that we arent told about this pain.

I have read all of the testimonials regarding stent removal. I had lithotripsy surgery ok n July 3 2018. Today is July 8th and my stent has fallen out. One of the uncommon things. Today is Sunday so I had to notify the on call doctor who advised me to go to the ER if my pain gets worse. I am experiencing pain similar to menstrual cramps. It has been 4 hrs since it came out so I am just waiting to see if the pain increases or I am unable to urinate. If remain the same I was advised to contact my doctor in the morning. Has any had this experience? It is making me a little nervous.

I had 5mm stone removed Monday of this week, 2 days ago, Thursday , I remover my stent and then that’s when the real pain began. Since then I have had about 8 pain flare episodes ranging from a 7 to a 10 level. I called my urologist, who’s been great and ordered another round of percocets . I never used any pain meds while stone was in. Ice packs work well also. So today I .still in bed minimizing my activity because after the pain last night I don’t want another episode. I had a stent in for 7 weeks after uretal reconstruction surgery. I was having my ovaries removed and the gyn discovered that the ureter, bladder and ovary had fused from adhesions and scar tissue from a prior gyn surgery 10 years earlier. He had to call a urologist into the OR. While the stent was in I felt like I had a constant UTI, with pain and burning during urination. That went away as soon as the stent was removed. About 15 minutes after the stent was removed I began having pain in my back. I had taken two Advil about 3 hours before the procedure, and took two more about 30 minutes after. The pain has ranged from discomfort to unbearable for two days. I’ve been taking Advil 3-4 times a day. I finally took one half of a Percocet that relieved the pain temporarily and put me to sleep for just two hours. I was given the Percocet for surgical pain and never needed to take one. The stent removal pain has been worse than any post surgical pain I’ve ever experienced. The stent was removed Thursday morning and it is now Saturday morning and the pain moved to the front left side of my body for a while but now seems to be in the back again. My surgeon was very good, but I am very annoyed at him for not warning me that the pain could be this scute. All he said is that I could feel pain on the bladder and blood in the urine, which I haven’t had at all. I wrote him a message telling him about this study and that I wished I would have been told to take Aleve 15 minutes before the removal procedure.

Update to my original post:
I did more research online and read that anticholinergic drugs can be effective in relieving renal pain. Two drugs in that class are Benadryl and Belladonna. I took one Aleve and one 200C dose of homeopathic belladonna. I put a heating pad on low and laid down in bed hoping the pain would stop. I fell asleep and woke up two hours later to ABSOLUTELY NO PAIN. The excruciating kidney pain was gone and has not returned since without taking anything more than that one dose of Aleve and homeopathic belladonna. I hope the same works for anyone in pain reading this.

I had two JJ stents for 35 days after cystscopic surgery to remove a bladder tumor
(Low grade / NMI).

The whole experience was not too much fun. What they thought was a kidney stone
Turned out to be a bladder tumor. I’ve now been knocked out a few times to have
Small bladder tumors removed. This last appointment I barely had a tumor back,
It was only .25mm. And I got placed from every 3 mint cystos to every 6 months

Anyways in regards to ureter pain after stent removal; I’ve had plenty. I had my agents removed on Oct 13 2017 and I still get ureter pain ever day

However some days I virtually have no pain. And other days it feels like I almost have a kidney stone.

I was on Xanax for a few years prior to finding out about my bladder cancer and my stents.

The pain after stent removal got so bad that i had to be put back on Xanax. It was the only thing that felt like it removed the pain. It was like a miracle when I went from intense pain everyday to basically none.

The Xanax still controls my ureter pain. And all my further renal cat scans and ultrasounds have shown NO ISSUES.

I see in Denver the 19th top best bladder cancer urologist in the world. She has a theory that 1 in every 200-500 people with stents develop something she coined “chronic uretic imdlalamation syndrome”. Basically unexplained ureter pain for up to several years after stent removal. Her theory is that since everyone’s bodies and ureters are different, some peoples bodies and nerves react more sensitively then others.

For me, this is proof with how well the Xanax controls the pain. Keep in mind that when I was off the Xanax and experiencing the pain, I was on a powerful opioid agnlsit painkiller called subutex. Even the subutex couldn’t really do anything for the ureter pain.

Only thing that could was breathing techniques, laying down in bed.

Until Xanax. Now I’m not condoning Xanax use and would hate to see someone become reliant on Xanax due to ureter pain like me.

But it is proof that this is a real condition. And there is relief out there. And for some, Xanax pros versus cons might be worth is so they can regain their quality of life.

Just a few of my 2cents. I actually have a lot more knowledge and insight on this if anyone has any questions or needs any further advice

Is it possible to have a smaller urethra & ureter than most? Everytime they’ve had to use a catheter on me & when I had a cystoscopy I have cried my eyes out. Now knowing this is it possible to have increased pain from the stent being in for a month, then removed, stone blasted & another stent placed for 3 days then removed….. any advice/insight would be greatly appreciated. I’m aware you’re not a Dr

Thought this would be a breeze after the pain of the kidney stone. NOPE😳
My dr left stent in 7 days. Had mine removed today. You know it’s bad when you ask the Dr. If it is going to hurt and she actually says YES!!!
Same symptoms as everyone else has posted. Feels like the worst UTI in history. Even the symptom of having pressure on bowels is accurate. Luckily I have pain meds from a previous surgery. I don’t know how you folks made it with just Tylenol. Props to you.
Hopefully this pain and irritation will pass soon. Good luck. I’m fixing to apply ice🤪

I had URS on 14th February, 2018. I discharged by the hospital after removal of uro bag in spite of my complaint of painful and frequent urination. Since I had total colostomy and ileostomy done the filling of bladder caused intestinal obstruction and had to be taken emergency department in a hospital. There uro bag was put again after gap of about 3 days of URS. At this time I had now contacted UTI and E-coli bacteria. After about 3 months uro bag and JJ stent were removed but E-coli didn’t settle even after months of allopathic treatment. Finally E-coli bacteria got settled with 3 month’s ayurvedic treatment. However this whole period I was suffering from back pain in the ureter stent area sometimes very severe. Now after about one year and two months after stent removal I’m experiencing severe pain these days and clueless what to do.

I will share my story, so you all know you are not alone. I had a kidney stone surgery 8 days ago, where the doctor lazered the stone (5-6cm stone that popped out of my kidney and was stuck at the top of the urether tube causing a kidney infection) and inserted the urethral stent under General anesthesia in the hospital. 7 days later, yesterday, the doctor removed the stent in the doctor’s office. Removal did not hurt. He put numbing gel at the area and removed the stent with the cystoscope. The procedure took 30 seconds. I felt good afterwards for about 1 hour, then the pain started, dull at first, in the back flank where my kidney is, then the pain intensified over the course of 5 hours, even worse pain than after the surgery, very similar to kidney stone attack pain. My doctors said the blood in the urine will stop very soon. It has so far, except a little speck of it on the toilet tissue. He said I may experience pain, spasms, twinges for a few days. Sometimes he said if the pain cannot be managed with pain meds that sometimes patients need to be admitted to the hospital to control then pain (also reading between the lines, probably also because of bacterial infection after stent removal). He put me on Cipro antibiotic for the next few days after stent removal. So the pain subsided a bit after a good 5 hours of suffering, trying to ride out the pain and not go to the ER), probably down to a 1 or 2 on the pain scale by using Tylenol Extra Strength, 1000mg every 4 hours. Currently the pain is bearable, but hard to sleep, small fits of sleep. I’m also using a heating pad on the back and another on the front as I also experienced pain spasms of my bladder. The front feels like period cramps (anther female like me will know what that feels like), the pain in the back like kidney stone pain, although right now as I type this, the pain has calmed down. In 5 weeks I have to go to for a renal ultrasound and on week 6, I see the doctor again, for I have 2 small stones (2-3cm) in my other kidney. I know this is my life, as I have been generating kidney stones as a teenager and now I’m in my 40s (I also suffer from Chron’s but thankfully it’s in remission).
After reading other patient comments here, it seems this is normal and I, and others, like me will have to endure. I read on Google that the pain can last 2-3 days but could go on for weeks.
I pray everyone here will find relief soon and can get back to their normal lives! We’ll get there!

Hi I had my stent out about 2 hours ago (weird sensation not a nice one)and first 30 mins -1hr were painless but after that wow pain worse then kidney stone itself. Paracetamol not touching it after 1hr ,will try some anti inflammatory .thought the worst of it was over .good luck to anyone els having this done.

Ok about 3-4 hours later found paracetamol does nothing but the anti inflammatory (ibuprofen) has really taken edge off . I have also used a hot water bottle and really think that also helps
Drunk lots of water and it’s sore to urinate but was advised to drink 2 Litres in order to flush kidneys out , definitely feeling bit better . Good luck to you all

Hi, Lori. I had the laser procedure for a quarter-inch stone on June 1st and stent until June 8th. An hour after stent removal I was back in the dr’s office in severe pain. Gave me a shot of toradol. Used hydrocodone off and on since.
Was getting better every day until midnight Saturday morning. Bladder spasm hit and was in severe pain for three hours. Pain lasted most of Saturday. I took another hydrocodone at bedtime after a long day of an anniversary party followed by being in a wedding. Woke up this morning okay but pain kept getting worse. Finally went to the ER. After an uncomfortable check to be sure I was voiding completely, I got another toradol shot and sent home with hyoscamine and hydrocodone. Very frustrating. I also have Crohn’s in remission.
I am praying this roller coaster ride ends soon.
Will be seeing my gastroenterologist this week. Hoping none of this upsets the digestive tract.
Third kidney stone but first one I couldnt pass.

I can echo absolutely everything you put here. My doctor definitely downplayed the amount of pain I would experience. I am so nauseous from the pain and am rethinking my entire life right now. I hope everyone gets well soon. Also, I’m wondering if a vaginal bacterial infection was caused by having this stint in for 4 days (I made them take it out at day 4!, I didn’t even want it in at time of surgery but he said he wouldn’t do the emergency surgery to laser the 9mm stone unless I agreed to let him put it in)

We have almost exact same story! In April I had a stone lodged just outside the kidney, set up huge infection, ended up in hospital. Later has laser lithotripsy on that side with stent. Removal of that stent followed by discomfort but manageable.

Many stones also on other side do we did same procedure on it st end of June. He said my right ureter is very narrow and was difficult to place stent in. I had it removed 2 days ago and since the pain has been excruciating as when I had stone lodged and infection. Uugh. Good you feel better.

I had a stent put in on the 18th of May. The Dr. tried to remove the stent today, but it was encrusted. Whatever they gave to attempt to numb me was not effective, and I felt everything during the 4 times the scope was inserted and removed, and while he was working the stent back and forth trying to break the acid free of the stent as he went. Also when it was reinserted into my bladder. I’m not sure it is ok, that he was trying to work it around to break off the acid. I have no idea how that is all supposed to go, but it just feels wrong that I went through that much pain, and end up where I started laying in a pool of urine. The pain was so bad that if the worst pain I had experienced to this point was a 1, the pain I had during the stent removal was a 10. They want me to come back next week for another crack at removing it under anesthesia while they have me there for ESWL for another small stone, but I get a bad feeling about the Dr. I discovered he had his liscense removed for 3 years because he had retrain. I would go elsewhere, but he is the only urologist at the VA. Would appreciate any comments that anyone can offer on the situation I described.

Totally agree with you Purpleberry! Robert I hope you were able to get the care from an experienced & knowledgeable Urologist who hadn’t had their licence removed. My story is that I actually had a 6mm kidney stone that was so stuck in mu ureter that my kidney started to swell. Went to E.R. was admitted to the Hospital & had the stent placed the next day under general anesthesia. The WORST pain I have ever been in. The next day I was crying for it to be taken out. Had been told stone was removed during procedure (untrue). Came home & basically laid on my left side in my bed for a month. I just had the stent removed on Friday however, I hadn’t passed the stone so they blasted it & put another stent in which they said i could remove Monday. It was removed Sunday evening. I’m still having extreme pain after I go and basically just while laying in bed. I have no clue what to do. Gonna try the ice pack see if that helps… I’m so over all of this pain etc.

I just had my kidney stent removed this afternoon, I went home and took a Tylenol extra strength. I am experiencing discomfort in my back and on the right kidney where the stent was. I had lymphoma and had a stent put in last August.
I did experience very big discomfort the entire time. Really pray that the discomfort will subside.
I was given antibiotic pill before the procedure and now I feel sick in my stomach also.

Had stone removed from tube last Thursday and a stent put in. Was in the ER Saturday due to extreme pain. I had bacteria in my urine so they gave me one antibiotic there at the ER and pain meds. Today is Tuesday and I removed the stent Was fine for about 20 mins….. now in extreame pain with blood in my urine again. Pain goes from kidney to my bladder. My “unit” is also in pain now. I take pain meds but as soon as they wear off the pain comes back 10 fold.

Hi I had my Stents removed last Friday 2 weeks after a double Uteroscopy. Scheduled for day surgery was in for 6 days. Got an infection so stents were in for another week, 2 in total. Immediate relief after the Stents removed lasted 10 minutes then Boom! The pain but me like a tonne of bricks and lasted approx 5 hours. I felt better 24 hrs laters but 48 hrs later had a fever and immense pain! Contacted the Dr another UTI!! So I’m now on more antibiotics (been 36 hours) and I’ve got awful genital pain and seeing blood! Will this ever end😩

I’m so sorry for what you’re going through…I thought I was in bad shape, but I guess I should count my small blessings! I had an obstructive 6mm stone for 2 months, failed lithotripsy and finally my urologist had to extract it. At that surgery, he placed a stent due to the swelling, and the pain of that stent was worse than the stone! My only reprieve has been oxybutinin, which only gives me total relief for an hour or two. I have realized that my bowels are a huge factor in this pain, which may not be considered in a normal workup. I’ve had a hysterectomy, so that may play a huge part in the buffering of my intestine/ureter contact. Gas or stool coming down my left sided colon sends me into a full spasm attack that I equate to labor that lasts for hours! Because all these meds are constipating, I knew that my bowels had to be a factor. I was so desperate for relief, that I tried an enema, to see if my hunch was correct. For me, it was the holy grail I needed to find a few hours of relief to actually get some sleep! I’m definitely not saying this will work for you, but desperate times call for desperate measures, and if you are prone to constipation, this may help you!

I just had 3 kidney stones removed from birth kidneys and had 2 stents for a week. They were removed 3 days ago and the pain is still horrible. Feels like I have a utibut they didn’t put me on any antibiotic sheet they removed them. Everyone keeps telling me I shouldn’t be having this pain, but I am. Don’t know who to turn to for help because I feel like no one is taking me seriously. I’ve been taking hydrocodone for more than 5 weeks because of these stones and want to be able to stop taking them. Any suggestions?

After PCNL procedure on 27th April with stent, the stent removal procedure was performed after three weeks on the 18th May n it was very conducive without any post operative pain and uirinary discomfort. After having a stent in for 4 months now, I am at breaking point. I hate peeing and getting jabbed in the side. Now I fear the removal process, my arms already feel weak as it is and one of my legs went numb, but do the hospital care? Nope. They all say, you need your GP. I don’t have a GP!!!

I had my stent put in 4/27/2018 during my ureteroscopy. After, the pain during urination was like a hot poker in my side each time. Stent removed 5/3/2018, while the extraction wasn’t too bad. The aftermath is like hell. My kidney spasms and that is “normal”. I am living on tylenol. Urination is no longer painful, now the pain is just constant. I am ready to feel better!

How are you feeling now that it is 20 days out after stent removal. I am having spasms and pain when i urinate and pressure to pee regular and I am 7 days out from kidney stone shock wave and 3 days out of stent removal.

i was bleeding after having kidney stents placed. when i called the doctor he said i would have to remove them myself. my follow up appointment was cancelled because i had already removed my stents at home. does this sound normal in anyway? they don’t have me scheduled to come back for 5 months. is this a big cause for concern because im freaking out.

Yeah when you remove stuff the doctors then have no obligation to help you. I was sick in hospital a few months ago, I self-discharged because I was so unhappy. Just before a doctor was about to see me but he was taking too long, so I wanted out. Once I did they told me to sign a piece of paper, and then said wait and the doctor will see you. He refused, and didn’t have to because I had then self discharged. So you taking out the stent allows the doctor to wash his hands of you. If anything happens again you’ll have to start from scratch.

I had a stent put in March 15th. My life was hell for a week’ I developed a UTI and had to stay in hospital. Once discharged I had stent In for a week Tylenol Extra Stenth was my best friend. I also had flomax and Pyridia to help alleviate the pain. I just got my stent removed yesterday and boy the pain was unbearable after removed the actual procedure was very quick thankfully but the aftermath not fun !!! I am now in ER on Toradal and Valium to relax the sting pain I feel while passing urine. At first i passed good amount then that UTI feeling of burning hit me at 2:30am! I now have strands of urine and waiting for UA lab results . These doctors make it seem so easy and it’s not! I’m a 32 year old female with kidney stone history . Eat right trust me ! Excerciss and find a good Urologist because in NYC they can sometimes be like mine is. Never going back to him . NYU seems to have a better team and I’ll follow up there. Good luck to all who have to endure this. My Vagina on the left side hurts man and I hope the pain subsided and I never get a stone again!

I just had a stent removed after they lasered the kidney stone, had the stent for 7 days. The pain after they removed the stent was unbelievable. I never felt such pain. My doctor at NYU told me I should have no pain. He was wrong. I have to follow up with a 24 hour urine test. I’m not happy with the surgeon I had, this doctor had zero compasssion and was very this isn’t a big deal type of responses. He had a total disregard to the pain with the stent and after. When calling his office they said they would call in a prescription and still n9 medication for the pain. Anyone else had intense pain after the removal of the stent? Or can recommend a good doc in nyc?

Yes me😢 just had my two stents removed 1.5 hrs ago. Removing was easy and not painfull. But an hour later it started to hurt at my kidney in de back. As If I have instructions and the kidney swells. Is this normal? Seems like all of you experience extreme pain after removal

I had a stent removed after my kidney stone surgery. It was removed on May 30th and there was no problem except for the stinging when I urinated. For a few days afterward I had an issue getting to the bathroom in time. But that has passed. Now I’m experiencing these painful cramps which we’re relieved with extra strength Tylenol. Today is June 2nd and after skipping a day the pain has returned. It feels like a period cramp. If this doesn’t get better by Monday I will place a call to the urologist.

I am having 2 Stents removed this morning. 1 was Put in on the 13th. After Kidneys stone removal another was put in. My side affects were Constant peeing and bowel movements at the same time. My life is hell. I had no troubles peeing before 1 was put in. Now it burns stings itches Spasms Very Uncomfortable. Removing my Kidneys it felt better than Stents.
